We are seeking a dedicated and skilled Clinical Nurse to join our Care Awaiting Placement South (CAPS) team!
About the role:
In this role, you will provide your nursing services, which have been consolidated by knowledge and experience, across a variety of health settings. As the Care Awaiting PlacementClinical Nurse, you will support and assist the Nurse Unit Manager of Care Awaiting Placement in the triage, assessment, coordination, and acceptance of patients from Southern Adelaide Local Health Network hospitals into Care Awaiting Placement South (CAPS).
The Care Awaiting PlacementClinical Nurse also assists with the ongoing monitoring of these patients whilst on these programs with close liaison with nursing staff of care facilities and medical staff providing care, as well as regular quality assurance visits to care facilities.
Requirements for the role:
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- Demonstrated competence in specialised care of the older person nursing practice in accordance with the relevant standards.
- Registered Nurse/Midwife with at least 3 years, full time equivalent, post registration experience.
- Experience in hospital substitution programs an advantage.
