The Role
The Clinical Nurse Specialist 2 (CNS2), Palliative & Aged Care, provides primary health care (advanced nursing clinical management, and clinical support) to palliative and aged care patient/s (state-wide) for Justice Health and Forensic Mental Health Network (the Network).
This position works closely with the Transitional/Nurse Practitioners – Palliative & Aged Care and the Multidisciplinary Team to develop, facilitate, implement, and evaluate treatment plans that effectively manage risks for Palliative Care/end of life care patients and aged care patients (male and female) with complex health needs.
The position holder will work within Justice Health and Forensic Mental Health Network (JHFMHN) and Ministry of Health policies, procedures and guidelines as well as Corrective Services NSW and Juvenile Justice NSW rules and regulations as set out in the relevant legislation.
