Community and Oral Health is leading health service delivery in the transitions of care space. Community and Oral Health (COH) has inpatient rehabilitation services, along with Hospital in the Home (inpatient care delivered in the patient's home) as well as a raft of services that deliver face to face services in the patient homes as part of our community service delivery. The transitions of care space also provide a variety of new and exciting opportunities for pharmacist to be at the forefront of patient centred care. About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Participate in quality and service improvement activities to continually improve patient care and experience
- Provide input in the development of pharmacy clinical practices, procedures and protocols
- Work collaboratively and effectively within not only the pharmacy team, but wider multidisciplinary team and with patients / carers
- Provide medications information and education to patients / carers to ensure ongoing medication safety
- Provide clinical education pertaining to routine tasks for students and clinical practice supervision to less experienced staff with the support of senior clinical staff
