About the Role
Provide Care Coordination to clients who have been referred to the service. The Community Assessor provides assessment for clients in their homes. This occurs in the context of the CGH Care Coordination framework and as a Regional Assessment Services under the Commonwealth and State Government.
The Community Care Coordination team provide assessment services in Wellington Shire (excluding Yarram area) for Commonwealth and State funded programs. The team provides the Regional Assessment Service for My Aged Care for people over 65 years (over 50 years for ATSI). For people under 65 years who are eligible for services, the team provides the assessment service for the Home & Community Care Program for Younger People and The Carers program. The team also provide reviews as required.
Duties
- As a Regional Assessment Service provider, the Community Assessor will be required to conduct the required assessment on line into the My Aged Care portal – this is the National Screening and Assessment Framework. As per the Statement of Attainment (SOA) 2 training for Home Support Assessors.
The Community Assessor will work closely with team members and managers in the area of their work, in order to support the highly integrated approach. This will require reaching into and working with the acute setting
- With the client at the centre of the process, document the required comprehensive care plan. This shall be documented as per CGH procedures which includes documentation on to the CGH Client Management System or the My Aged Care portal if for a Commonwealth Home Support Program funded service. This is a key deliverable of the Community Assessor process
- Approach assessment using a wellness and reablement approach
- Minimise duplication of effort in the assessment process
- Enable assessment and care planning to be undertaken at the earliest possible time and consistent with the CGHS person centred, goal based approach
About You
Statement of Attainment (SOA)2 training for Home Support Assessors, covering the following units of competency (must be successfully completed in the first 6 months of appointment):
Facilitate the empowerment of Older People (Unit 1 CHCAGE001)- Conduct Individual Assessments (Unit 2 CHCCCS005)
- Respond to Client Needs (Unit 3 CHCCS016)
- Experience and proven accuracy using electronic office and specialised computer applications including data base applications
