Community Care Coordinator – North Midlands
Coordinates home and community-based care services for clients and carers within the North Midlands Region, focusing on enabling, optimising independence, respecting choice and ensuring that clients can remain in their homes and be part of the community.
Key responsibilities:
- Coordinates and assists with the daily operations of individualised Community Care services.
- Facilitates referrals of new clients to appropriate organisations and facilitates assessments when required and coordinates the delivery of services ensuring client needs are met.
- Ensures individualised client care plan reviews take place regularly and are updated to reflect client’s needs, health service policy and Aged Care quality standards.
- Collects monthly, quarterly and bi-annual client and activity data, including reporting and submissions to management.
- Attends to invoices, collects and receipts service payments, ensuring that services are documented and delivered against rosters.
