About the role
Full time permanent opportunity, based in Milton, QLD. Reporting directly to the Team Leader Client Services, you will be responsible for:
- Building relationships with local stakeholders/key referrers and collaborate with other Gateway providers.
- Increasing carer referrals and access to internal programs and services provided by Dementia Australia.
- Leveraging internal and external resources to develop responses to consumer needs to address barriers to accessing services.
- Facilitating improved referral pathways for carers and their families.
- Representing Dementia Australia in a variety of settings, interagency meetings, stakeholder meetings and at public speaking engagements.
- Frequent intra-state travel including overnights will be required.

About you
To be successful in this role, we are looking for someone with the following:
- Tertiary qualification in a relevant discipline highly desirable, (health, allied health, social services, education, management sectors) and/or knowledge and/or experience of community development or working in a similar role.
- Experience in liaising with and developing relationships and partnerships with service providers and other agencies.
- Experience in group facilitation and providing community information and education and supporting community capacity building.
- Demonstrated experience in effectively collaborating with a wide variety of community groups and individuals to achieve practical and sustainable mutually beneficial outcomes.
- Knowledge of dementia and its impact on people diagnosed with dementia, families and carers, particularly around best practice carer supports that enable carers to sustain their role as a carer and enhance their own wellbeing and identity.
- Demonstrated ability to work independently with minimal supervision and contribute effectively as a team member.
- Valid driver?s licence for frequent travel is required.
