The Community Nurse reports to the Community Services Manager and will ensure our community clients' clinical care needs are met. They will also work as a Registered Nurse in both Community Services Areas. They will also oversee and provide clinical leadership and guidance to the practice nurses and any clinical staff in the community area.
This includes overseeing our Homecare, NDIS, Palliative care, private services and CHSP programs in all matters relating to standard of care and legislative requirements for clinical care.
The Community Nurse will build professional relationships within our community and with other stakeholders to understand our current and future clients’ needs and develop ways that we can help to support and empower clients to achieve the things that matter most to them.

- Minimum of one year’s nursing experience, preferably in a community-based role.
- NDIS worker screening or willingness to complete
- Bachelor of Nursing with Current APHRA registration
- Unrestricted Australian Driver’s License
- Knowledge of current immunisation schedules, Diabetes management, asthma management, enhanced primary care items, infection control, CPR and emergency resuscitation techniques, equipment & drugs.
- Strong understanding of Homecare, NDIS and CHSP programs or the ability to acquire knowledge
