ABOUT THE ROLE

Gateway Health are excited to be launching the new CP@ (Community Paramedic) program within Gateway Health. This is an innovative, place based approach to supporting community members who are disconnected from their community and the healthcare system.

The program will primarily focus on underserviced rural/regional areas and with the people who need the service the mostThe CP@ program is a chronic disease prevention, management, and health promotion service. The aim of the CP@ is to support members of the community manage their health outside of the emergency setting.

A typical day will involve you:

  • Conduct several evidence-based health assessments
  • Provide referrals to primary care and community resources
  • Assist clients in navigating the health care system
  • Provide clients with tailored health education
  • Engage participants in healthy lifestyle discussions
  • Set health goals with participants

Internationally, the program has demonstrated a significant reduction in emergency call outs, improved management of chronic conditions, and improved empowerment and social connectedness for people engaged with this service.

ABOUT YOU

We are currently seeking highly skilled Paramedics who have a strong clinical background and experience working within the community health industry. 

To be successful in this role you will bring with you:

  •  A degree in Paramedic Practice
  • Be registered as a Paramedic with the Australian Health Practitioners Regulatory Agency (AHPRA).
  • Excellent communication skills, in particular the ability to communicate sensitively in a cross-cultural environment.
  • Proven clinical assessment skills
