Based in the Mid-West WA (Geraldton), this role assists clients by working collaboratively with other health professionals and services. As their Physiotherapist you will co-ordinate and deliver client centred care plans to clients in their own home helping them to achieve improved Physical, Occupational and Health goals and have the best life possible at home.
Full time or part time will be considered. Part time will be minimum 3 days per week (45.6 hours per fortnight)
Key Responsibilities- A demonstrated understanding of comprehensive & holistic assessments of clients to determine an appropriate care plan
- Plan and flexibly deliver physiotherapy services to clients in need in their own homes and ensure they are given the appropriate care
- Maintain liaison with families/care givers to ensure ongoing care for the client by providing support, counselling and education on client management and treatment
- Maintain adequate and timely documentation records to enable review of clinical activity and ongoing client care
- Prepare clinical reports as required and keep attendance records to enable review of training activity
- Ability to develop relationships with individuals from diverse cultural backgrounds
