Our client is a not-for-profit community-based service who are dedicated to enriching the lives of their clients by supporting them to maintain independence at home or assisting them with access to resources &/or residential care.
About the role:
- Introducing the service to clients, raising awareness of the support services available, and managing a program of ongoing information and feedback sessions.
- Overseeing the case management aspects of the service and able to act in consultation with Management to put in place the necessary administrative systems and policies.
- Consulting with and assisting clients to identify service goals and service needs.
- Conducting ongoing case management services.
- Build sustainable, reliable relationships with the clients providing confidence & support.
- Registered Nurse with a combined 6years experience across acute care & delivery of community care.
- Familiar with conducting health assessments in the home & identifying clinical needs & escalating clinical deterioration to the appropriate service.
- Demonstrated case management experience in planning, coordinating and delivering services to our older adults.
- An in-depth understanding of the Home Care &/or Commonwealth Home Support Packages.
- Ability to prioritize, good organizational skills, be time-efficient, and have good computer skills
- Full Driver's License (Company Car Provided)
