About the Role
- Provide support, information and advice to NDIS participants and evaluate and coordinate the services of welfare and community service agencies.
- Interviewing clients and assessing their needs and conditions
- Prepare, manage and implement the participant’s plan
- Liaise with and report as required to NDIS in relation to participants and their plans
- Provide direct assistance to NDIS participants and their families
- Provide NDIS advise to participant and families
- Achieve the outcomes identified in the participant’s plan
- Connect the participant to services and funded support.
- Going above and beyond to strengthen our participants’ abilities to coordinate and implement supports and participate fully in the community.
- Assist participants to work towards their goals in their NDIS plan
- Ensure the participants' plan is implemented in accordance with the allocated budget.
- Liaising with internal and external services to maximize participant outcomes
- Building capacity in individuals to manage their own supports
- Monitoring and reviewing plans and goals in consultation with participants
- Providing support and assistance as appropriate to relevant others/stakeholders to promote awareness of community resources and services
- Establishing and developing strong relationships with NDIS, community, and mainstream supports
- Preparing submissions for funding and resources, and reports to government bodies and other agencies
Requirements:
- Must have an allied health bachelor's degree or above in social work, psychology, developmental education or other relevant field
- 5+ years’ management experiences in health or related field
- Must have a Department of Human Services Disability Services Employment Screening check or NDIS Workers Clearance, in conjunction with a Working with Children Check (Essential) prior to applying.
- Current Australian driver licence with a roadworthy vehicle (Essential). Must be willing to use their own vehicle for work related use, including home visits and attending stakeholder meetings.
- Strong communication skills, both verbal and written; bilingual or multilingual is preferred.
- Excellent time management, budgeting and administrative skills.

- Good presentation and a professional attitude
