About Support Base:
Support Base is a local provider of Quality NDIS services across Bendigo and surrounding areas plus Melbourne. Our services include Support Coordination, Specialist support coordination and Psychosocial Recovery Coach capacity building supports.
We also provide ‘Daily Living’ support plus ‘Social and Community’ supports.
Support Base offers support to participants in their homes and in their local communities. Our focus is on a strength based support which fosters positivity, independence and offers genuine care and practical support.
Our Management are very experienced and approachable and have approx. 30 years of experience working across the disability and welfare sectors.

Ideally, you will have flexibility to work most weekdays and some weekends (if required) as well as have the flexibility to work additional shifts (if required).

Tasks & responsibilities:
The Support Worker role can vary depending on the clients’ support needs and support requirements, the duties on an average support shift may include (but not limited to):
o Expereince in working with Young People and Mature Aged People
o Physical assistance to participate in activities.
o Mental Health experience would be an advantage.
o Transporting clients in vehicles to various locations or assistance to use public transport.
o Support to attend places of interest. including community events etc.
o Support to attend appointments and regular activities of the Client.
o Perform the necessary duties to ensure that the premises and vehicles are kept clean and tidy.
o Providing emotional support to clients by engaging in conversations and using your active listening to address / support their needs.
o Collaborate with other Support Workers / managers to promote team support, gain advice and facilitate best practices
o Accurately complete administrative duties as required. This may include reading / writing / typing / report writing:
o Document progress notes, and maintain accurate records about the Client’s progress.
o Communication books
o Incident reports
o Complete vehicle logs
o Timesheets
o Other required documentation
Qualifications Required:
- Current Certificate for First Aid (HLTAID011) & Cardiopulmonary Resuscitation (CPR) (HLTAID009)
- Cert III Individual Support (Disability) or Cert IV in Disability (desirable) or similair Community Servcies etc
- Cert IV in Mental Health or similar would be and advantage
- Full Current Police and Historical Criminal Records Check
- Current Employee WWCC
Additional Requirements:
- Current driver's licence (Victorian).
- Covid19 Vaccinations - (proof required).
- Current NDIS Worker Screening Check.
- Current Motor Vehicle Insurance policy.
- Completion of NDIS Worker orientation module.
- Modern releiable vehicle - attractive km reimbursement.
Key Selection Criteria:
- A demonstrated understanding of providing daily support as directed by management and in line with the clients NDIS Plan.
- Possess strong interpersonal and communication skills with a range of people in a variety of situations, including stakeholders.
- Ability to identify and implement creative solutions to meet the clients support needs.
- Demonstrated ability to address minor barriers and be well organised with exceptional time management skills.

remuneration:
SCHADS award is applicable - Level is dependant on qualifications, skills and experience - PLUS 11% Superannuation and other entitlements.
