Our team of healthcare professionals specialises in continence management and is committed to delivering services that align with the NDIS standards.
We are looking for experienced Support Workers, Registered Nurses and Enrolled Nurses to join our growing team in Brisbane on a casual basis. 15 to 20 hours a week.
Support Worker:- Ensure that tasks documented in the Client Care Plan or by the Coordinators are delivered
- Assist clients to maintain independence by delivery tasks in a timely way
- Provide prompting / supervision and assistance with client medication as determined by a Registered Nurse and documented in the client’s care plan
- Assist clients with personal cares including showering, bathing, toileting, haircare, skincare and grooming
- Assist clients with dressing and undressing
- Assist clients with household tasks such as cleaning (vacuuming, dusting, kitchen, bathrooms)
- Assist clients with laundry (washing clothing, towels, linen)
- Assist clients with changing / making beds
- Assist clients with meal preparation and cooking
- Follow any nutritional guidelines in the client’s care plan
Requirements:
- A background working in or understanding of the health, disability and/or home and community sector
- Hold or commit to work towards a Certificate in Health Wellbeing
- Knowledge of health and disability, short and long term conditions
- Delivering individualised clinically based nursing care such as wound care, catheter care, PEG changes / care, bowel care and medication management to customers within their own home environment
- Conducting nursing assessments, developing care / support plans & doing home visits
- Providing clinical nursing advice and support to our participants and their families
Enrolled Nurse:
- Travelling between participant’s homes and providing individual and personalised support to them in accordance with their support plans
- Building rapport with your participant and their supports (informal and formal)
- Working under direct and indirect supervision of an RN
- Covering the support worker if needed
- Performing urinary catheter insertions, changes, and removals according to established protocols and infection control guidelines for both indwelling urinary catheters and suprapubic urinary catheters.
- Assessing and monitoring catheter-related complications, such as infections, blockages, or leakage.
- Collaborating with healthcare professionals to develop care plans for patients requiring long-term catheterisation.
- Educating patients, caregivers, and families on catheter care, hygiene, and recognising signs of potential complications
Requirements:
- Bachelor of Nursing Degree or Nursing Qualifcation (for nurses)
- Current AHPRA Nursing Registration (Nurses)
- Complete an NDIS Worker Screening Check
- Professional Indemnity up to $10m
- Public Liability Insurance up to $10m
- Current Driver’s license
- Reliable registered and insured vehicle
- Annual CPR certificate and First Aid Certificate
- Annual Immunisation Certificate (including COVID immunisation history)
- Computer with internet access
