An exciting opportunity exists to work for one of South West Queensland's most dynamic and community responsive organisations. Lifeline Darling Downs and South West Queensland Limited (LDDSWQL) has been serving the community of Toowoomba, and the wider South Western Queensland area for over 40 years.
As our Family Response Facilitator you will be responsible for quality service delivery to children, young people, families and the community within the Family and Child Connect service (FaCC) and actively engages with individuals and families to assess the response type required and action accordingly.
As our Family Response Facilitator, your duties and key responsibilities will be:
• Gather sufficient information from the individual or family members to inform assessment and apply the Queensland Child Protection Guide, Practice Framework & Common Assessment Tools to assess the response type required and action accordingly.
• Consult with relevant stakeholders as necessary to assess safety and risk factors for children with a view to determining response type.
• Consultation as necessary where family violence is indicated with a view to assessing safety factors and associated response implications.
• Provide information, advice, referral and linkage to relevant services, as appropriate to the assessed level of need.
• Through outreach, engage with families with multiple high level needs, and provide brief initial case management and intervention until the assessment and referral is finalised.
• Maintain accurate records, data entry requirements and Assist with the release of personal, private or confidential information in line with the Lifeline Privacy Policy, privacy legislation and the Child Protection Act, and in consultation with the Manager – Human Services
• Work collaboratively with relevant stakeholders to prepare, undertake and maintain external licensing/accreditation requirements.
• Relevant Tertiary Qualifications, Diploma or Certificate in the fields related to this area of work e.g. Child Protection, Human Services, Social Work, Community services and/or Psychology or equivalent qualifications.
• Experience in strengths-based casework with complex families
• The ability to work confidently and effectively across a range of service providers, both government and non-government, to negotiate high quality service provision to families who have complex issues.
• Excellent time management skills and the ability to adapt to challenging work environments, resulting in efficient use of resources, sound outcomes for families and quality documentation for evaluation.
• Advanced communication skills both verbal and written, with an emphasis on effective interpersonal skills, negotiation and conflict resolution including the ability to communicate sensitively and effectively culturally diverse individuals
• Current Queensland “C” class drivers licence
• Working with Children Blue Card
