- Previous experience in coordinating and reviewing Chronic Disease Management plans in a GP setting
- Passion and experience in supporting elderly patients in their health journey by undertaking Health Assessments, including home visits.
- Able to use initiative to manage own workload and optimise bookings
- Experience using Best Practice Software.
- Strong work ethic and able to commit to permanent role within the clinic.
- Enthusiastic for continued learning and teaching.
- Strong communication skills including accurate and comprehensive documentation.
- The ability to cover annual leave and take on additional shifts as required is essential
- Current AHPRA registration, Police Clearance, CPR certificate, SA child/vulnerable person clearance
Core skills:
- Health Assessment and Care Planning
- Wound care management
- Immunisation (children, adults), and knowledge cold chain storage and vaccine management
- ECG, spirometry, ear syringing, iron infusions.
- Assisting in minor surgery clinics and women’s contraceptive procedures (coil/implant)
- It is expected that you will contribute to the recall system and development of our clinical protocols, working towards ongoing accreditation, and summation of medical records
- In addition, at busy times to help in other areas of the business such as reception/admin duties
