What You Will Be Doing
We require a Permanent Part time Geriatrician Physician (0.2 FTE) to work across Frankston Hospital and Rosebud Hospital - Geriatric Oncology outpatient clinics . The Rehabilitation Physician will be working with an Oncologist , Geriatrician and Project Manager.
The purpose of this clinic is for implementation of a geriatric oncology model of care for new cancer patients receiving care from Peninsula Health who are aged 70 years and older.
The core model represents an extended multidisciplinary team providing care for the older cancer patient by streaming to either a comprehensive geriatric assessment (CGA) or referral to rehabilitation medicine based on results an initial geriatric screen by Geriatric - 8 (G8) screening tool. Depending on the outcomes of the assessment process, cancer care planning and service delivery specific to the needs of this patient cohort will occur in parallel with the delivery of anti-cancer treatment and in close partnership with palliative care, GPs, allied health and community-based services.
What You Need
We anticipate you will have:

• Medical Practitioner Registered in Australia.
• Fellow of Australasian Faculty of Rehabilitation Medicine (FAFRM).
• Relevant track record and experience in clinical management.
• Sound clinical knowledge, skills and experience in contemporary medical and evidence-based practice.
• Ability to work as an effective member of a multidisciplinary clinical team
• Teaching skills.
• Patient safety and quality review knowledge and skills.
