These positions will be based at our Kingston Centre site initially, however you may be required to work at any of the Monash Health sites.
About GEM@Home:
The GEM@Home Program provides care for individuals with complex medical, functional and often cognitive issues who require comprehensive, Geriatrician led, multidisciplinary assessment and optimisation. The service is currently accepting patients within the Monash Health catchment and within 30 minutes of the Kingston Centre.
Patients being referred to the GEM@Home Program must not require inpatient hospital level treatment and must reside in a suitable home environment. Depending on the patient and their needs, they may also require family/carers to provide supervision or hands on support whilst admitted to the GEM@Home Program.
The GEM@Home Program provides once daily visits from at least one member of the multidisciplinary team, but the Program is not able to provide 24/7 care. Individuals admitted the GEM@Home program are considered hospital inpatients and are therefore unable to utilise community or outpatient services whilst on the program ie. Home care packages or Medicare billed outpatient appointments.
A Day in the Life:
In joining our team as a Grade 2 Occupational Therapist within the GEM@Home Program, you will leading delegated responsibilities to provide comprehensive Occupational Therapy and best practice patient centred care, with the aims of:
improving patient outcomes;
achieving maximal operational effectiveness; and
facilitating sustainable discharge plans (in a fast-paced environment) for patients admitted to this Program.
Your key responsibilities will include:
utilising your specialist knowledge and skills in client assessment, treatment and coordination of services to work towards optimising health, well-being and participation
working independently within approved scope of practice to provide safe, evidence-based assessment, outcome measures, therapeutic care and/or discharge planning according to organisational policies and procedures in order to achieve optimal care
providing specialist knowledge, advice or guidance to other clinicians to support safe and effective consumer centred care
working collaboratively with consumers, their families and/or significant others and other stakeholders to establish a patient/client focussed multidisciplinary management plan, consistent with currently accepted professional standards of practice and evidenced based practice
working collaboratively with the multidisciplinary team to deliver safe consumer-focussed care, providing specialist knowledge or depth of experience
using specialist clinical reasoning and guiding others to prioritise and fairly distribute workloads in collaboration with the team as required
What You Bring:
demonstrated specialised knowledge and skills in Occupational Therapy assessment and intervention
usually at least several years of clinical experience
strong clinical Occupational Therapy experience in rehabilitation and community care
demonstrated well-developed oral and written communication skills
proven ability to engage in reflective practice to support specialist clinical care delivery
demonstrated experience in contributing and responding to change ideas and supporting others through this process, using specialist knowledge
ability to work autonomously and collaboratively with the multidisciplinary team to develop effective clinical outcomes for patients
What You Need:
successful completion of a Bachelor of Occupational Therapy, Bachelor of Applied Science (Occupational Therapy) or recognised equivalent degree
current registration with the Australian Health Practitioner Regulation Agency (AHPRA) - Occupational Therapy Board of Australia (practicing, no restrictions)
current Driver’s Licence without restrictions

These roles are supported by the GEM@Home Nurse Manager, Allied Health Clinical Lead, and Senior Occupational Therapist (Grade 3). The successful applicants will function as a member of a dynamic multi-disciplinary team and will work closely with team members to promote an integrated and holistic service. They will have a strong emphasis on inter-professional leadership, teamwork and outstanding communication resulting in positive client outcomes.
