About the role
An exciting opportunity is available for an enthusiastic Occupational Therapist to join Eastern Health working across Continuing Care - a new and innovative subacute service specialising in the progression of care with a particular focus on patients with disabilities.
The position will be providing comprehensive OT assessment, clinical reasoning, complex decision making and discharge planning for NDIS participants experiencing barriers to discharge from hospital. The successful applicant will work closely with the multidisciplinary team to understand the person within the context of their environment and occupations to provide supporting evidence and recommendations to facilitate community transition. Where indicated, this position will also work closely with key stakeholders to support identification of appropriate care models and discharge environments to support community transition for people with disability.
About you:
To be successful in this role you should possess:
• Current occupational therapy registration with AHPRA
• Demonstrated clinical expertise in inpatient occupational therapy assessment, clinical reasoning and discharge planning for patients experiencing complex disability
• Excellent verbal and written communication skills
• Highly developed organisation and self-management skills
• Strong commitment to work collaboratively within a multidisciplinary team and with internal/external stakeholders
• Awareness of the NDIS and NDIS specific processes
• Awareness of NDIS and non NDIS housing models
• Established skills in assessment of home environment and interim assistive technology to promote independence and community transition
• Commitment to creative problem solving and risk management approaches to promote community transition
