Together, we enable older people to maintain their individuality, providing what is needed to support a fulfilling life.THE ROLEThe primary responsibility of the Group Activity Manager is to ensure the management of CHSP services to our clients, their families and carers using a wellness approach, whilst embracing a client centered methodology that demonstrates and promotes Amana Living's Philosophy of Care and core values. This role is a Full-Time role (Monday - Friday 8am-4pm)Key responsibilitiesEnsuring coordination of services and level of contact with clients is in accordance with CHSP and Amana Living guidelines and where needed contact appropriate referrals to provide further support services

• Acting as a first point of contact for all CHSP Kites Program and Shopper bus referrals within the designated geographical area from My Aged Care (MAC) to Amana Living clients and their families

• Meeting clients and conducting the Health and Safety and environmental check at the initial referral and then annually

• Providing telephone and where appropriate face to face support of clients and carers within the allocated geographical areas, with a focus on wellness and enabling independence as per CHSP guidelines

• Ensuring staff work to the MAC developed care plan summary

• Maintaining and updating in a timely manner all client case notes and client files both hard copy and electronic

• Generating weekly rosters through Comcare rostering

• Attending and actively participating in compulsory training sessions and team meetings

• Contributing to the continuous improvement of own work practices and acting to enhance the professional development of self

• Performing all duties and interacting with management, staff, clients and their families, external service providers and other health professionals and any other person in the work environment in accordance with Amana Living Values

• All other duties or projects as directed by your manager

• Responding to client complaints, concerns and client invoice queries in a proactive and professional manner, investigating where necessary, resolving in a timely manner and reporting the matter appropriately, escalating any issues to the Operations Manager as needed

• Providing after hours on-call duties according to the rostered schedule

• Identifying, initiating and coordinating quality improvement activities in relation to client care including quality audits to ensure compliance

• In collaboration with Kites Program Assistant, ensure quarterly program planner is completed in a timely manner. Overseeing the trip schedules and venue safety assessments are completed.

ABOUT YOUThe successful candidate will be highly organised with a sensitive and proactive approach to client care and service delivery, collaborating with the Kites Program Assistant and other Home Care Staff, ensuring that care is delivered in accordance to CHSP guidelines.Competencies & BehavioursStrong time management and organisational skillsProficient in Microsoft Office applications and Client Management databasesStrong problem solving and complaint resolution skills and the ability to take initiative with decision makingExcellent teamwork and liaison skillsEssential Criteria

• Evidence of COVID-19 and 2024 Flu vaccinations
• Valid 'C' Class Drivers Licence
• Ability to obtain a National Police Clearance (less than 6-months validity)
• Exposure to Aged Care sector (desirable)
• Tertiary qualifications of Certificate IV in Aged Care and/or Community Care (desirable)
