About your opportunity Permanent full-time or part-time / hours negotiable / Multiple positions available The primary purpose of the Health Worker – Advanced is to facilitate effective, coordinated and culturally capable health services for Aboriginal and Torres Strait Islander people living in the Townsville Correctional facilities. They will undertake effective discharge planning for patients to facilitate successful transition from prison, back into the community. They will direct patient care using clinical skills appropriate for the delivery of health assessments and supporting the Nurse Navigator to identify and coordinate complex care needs for high risk prisoners. You will provide cultural leadership to health professionals and provide health education and support to the patients receiving health care.
