The Opportunity
We are seeking a Case Manager for seven participants who either live in their home under SIL, or an in-home arrangement. We also have an eye on expanding into ILO so the case manger will champion leadership into this new service direction. Our focus is on always improving the quality of what we are already providing while expanding our reach to make a difference in more lives.
Key Responsibilities:
- Provide planning and support for participants and their networks through periods of transition with a focus on achieving stable living arrangements.
- Identify opportunities for growth and development, collaborating with stakeholders to establish new out of home care services that meet the needs of the community.
- Manage projects related to property search, housemate selection, host connections, form completion, budgeting, and support coordination.
- Leading support worker teams by providing guidance, support, and motivation to ensure a cohesive and effective workforce.
- Work closely with internal teams, external agencies, and community partners to strengthen collaborative relationships and improve service delivery.
- Ensure compliance with industry regulations, licensing requirements, and accreditation standards.
What you bring
- Resilience and smarts, with a strong sense of alignment to our values and mission.
- At least 3 years of NDIS experience in support coordination, SIL or a Home and Living type of service. Formal qualification in social work an advantage.
- Knowledge of the NDIS sector, particularly SIL, SDA & ILO
- At least intermediate IT skills with demonstrated ability to confidently generalize across different platforms.
- A passionate for building strong relationships and being influential in your approach.
