The exciting opportunity:
- Clinical assessment and planning of Home Care Program for a prestigious provider 20 minutes North or Perth’s CBD
- Oversee the assessment, support care planning, clinical governance, and uphold the expected standard of care for customers in accordance with Home Care legislation
- Ensure high quality care delivery that aligns with compliance and customer expectations
- Collaborate with health professionals to provide individualised care respecting client choice
- Manage obligations, compliance, and contractual outcomes for brokers and contractors
- Contribute to training new team members and support workers for effective documentation
- Propose changes to clinical procedures, processes, and policies for continuous improvement
- Develop strategies and succession plans for own role and service provision staff
- Actively participate in audits and reviews aligned with Quality and Safety systems
- Avoid corporate bureaucracy and red tape by joining a small aged care provider
- Opportunities to drive care quality and ensure compliance and governance standards towards Home Care services
- Be guided and supported by your dedicated 3D Recruit Career Consultant, assisting with CV creation, interview preparation and career advice
- $50 – $52 an hour + super
- Must be an AHPRA Registered Nurse
- Proven track record in aged care with the ability to conduct assessments, devise care plans execute service provision, and manage administration
- Exceptional customer service, superb interpersonal and communication skills with the ability to build rapport with and engage customers and families
- Innovative, open minded, emotionally intelligent and personable
