About the role and where you will be working:

The Home Care Service Coordinator provides excellence in applying a wellness and reablement approach and coordinate services  for clients to achieve their goals and improve their health and wellbeing outcomes. To work collaboratively with clients, internal and external stakeholders to assess, plan and facilitate the delivery of care to clients considering their assessed care needs strengths and goals.

This position is located at our Bran Nue Dae centre, co-located with Germanus Kent House in Broome (2,044.5 km north of Perth)

We’ll also be asking you to:

  • Provide leadership and support to the Home Care staff.
  • Work in conjunction with Home Care Management to plan and implement responses to client care needs, Support Staff and scheduling guidelines.
  • Monitor relationships and outcomes of relationships with wellness partners, clinical leads, allied health team and scheduling team to ensure the delivery of high quality flexible and accountable Home Care Services.
  • Work alongside Service Delivery Team Leader to ensure efficiencies in service delivery and scheduling of frontline service delivery staff to align to EBA/Award conditions.
  • Effectively investigate and resolve clients concerns and complaints in a proactive manner in relation to frontline service delivery staff in a collaborative model with all levels of Home Care management.
  • Provide data and reports as required for programs, and to assist with workforce development
  • Ensure policies, procedures and work systems are maintained and all quality and accreditation guidelines are met.
  • Regularly attend the meetings as required both external and internal including case management

To be considered for this role, you’ll need:

  • Experience leading a small team
  • Experience working with Aged Care or similar industry, particularly managing client outcome-based program funding e.g. CHSP, HCP etc.
  • Experience working within Indigenous communities/sector
  • Ability to work with internal/external stakeholders to achieve program and client goals
