About the role
The Home Care Package Onboarding Specialist at Uniting NSW/ACT will be responsible for visiting, converting, and onboarding new clients for Home Care Packages within the Metropolitan Sydney area. It is a full time 6-month contract with the intention to extend with the office base in Parramatta.
This role will involve conducting assessments, creating care plans, managing budgets, and provisioning interim services for clients.
You will be an integral member of the Senior Services Team through the following:
- Client Onboarding: Conduct comprehensive visits to prospective clients referred by the Business Support Team to explain the Home Care Package offered by Uniting.
- Assessment and Care Planning: Perform detailed assessments of client needs, preferences, and goals. Develop tailored care plans in collaboration with clients and their families to address individual requirements.
- Budget Management: Assist clients in understanding and managing their Home Care Package budgets, ensuring transparency and effective allocation of funds.
- Provision of Interim Services: Implement interim services as necessary to support clients while awaiting full-service commencement.
- Collaboration and Handover: Work closely with neighbourhood coordinators to seamlessly hand over clients, ensuring a smooth transition for ongoing care provision.
