CCA New South Wales is a proactive, community based organisation operating predominantly in the Northern Sydney District, providing a range of support and service programs to people living in our community.
About the role:
Your new role as the Aged and Disability Intake and Assessment Officer will be offered as a permanent Full time contract, based at our Head Office, 12 Lakeside Rd, Eastwood NSW 2122.
Purpose of the position:
- To effectively liaise and support the elderly and people with a disability and their carers through ADS services using My Aged Care and NDIS systems.
- Intake, onboard and assess clients.
- Maintain paperwork and client management system to ensure compliance.
- Assist Manager of Community Services in data collection, reporting, database management and compliance.
- Assist Manager of Community Services with administration and future planning
- Participate in client acquisition campaigns
Key Responsibilities:
- To ensure the effective running of ADS services providing background support including ensuring compliance.
- Management of referrals from my aged care, other service providers and self-referrals.
- Engage with clients on care plans and encourage service uptake with a strengths-based approach.
- Conduct home visits to complete assessments
- Assist Manager of Community Services with administration
- Participate in client acquisition campaigns
- To collect data and maintain paperwork, database and case note accurately.
- To promote the program within the community and to other service providers and attend organisational meetings.
- To support clients to access My Aged Care
To be considered for this role the candidate will have:
- Full Covid-19 vaccination
- Minimum – Bachelor’s degree or experience in the relevant field.
- Current Driver’s License and access to a reliable vehicle.
- Extensive knowledge of the community sector
- Demonstrable ability to work in a team and independently
- Excellent written and oral communication skills.
- Active listening and engagement skills with frail aged and people with a disability
- Ability to effectively interact with people at all levels and from diverse backgrounds.
- Strong time management skills, commitment to meeting deadlines and the ability to cope effectively with a sometimes pressured working environment
- Intermediate computer skills – Microsoft Office and client management systems
