About the program
The Psychosocial Recovery Service (PRS) is a program funded by the Commonwealth Government and commissioned through Primary Health Networks to 'provide psychosocial support services to assist people with severe mental illness, resulting in a reduction in psychosocial functional capacity.
The overall objectives of the PRS measure are to:
support people with a severe mental illness and associated psychosocial functional impairment who are not more appropriately supported through the NDIS; and
reduce the avoidable need for more intense and acute health services and enhance appropriate/optimal use of the health system.
About the opportunity
This role involves conducting Intake and assessments using validated tools, developing short-term action plans within a stepped care model, and providing service navigation and referrals to internal and external resources. The individual in this role facilitates group-based activities to enhance life skills, offers person-centered recovery and trauma-informed supports, and assists in accessing the National Disability Insurance Scheme (NDIS). Additionally, the role includes identifying and supporting families, friends, and carers, promoting engagement in peer support and education programs, facilitating access to primary health services, and providing ongoing support and contact with waitlists. The overall focus is on addressing the diverse needs of participants, fostering community connections, and promoting well-being.
To be successful in this role, you will need:
Community Engagement: Establish and maintain relationships with the community, local service providers, and organizations, including first responders.
Assessment and Short-Term Support: Conduct initial intake assessments and provide short-term support for immediate needs without requiring full referrals to waitlists.
Advocacy and Referral: Advocate with other agencies to access assistance when individuals' needs are better met elsewhere and refer family and carers to appropriate sources.
Representation and Evaluation: Represent Wellways in various settings, contribute to evaluating community engagement programs, and identify family and carer needs.
Co-Design and Community Events: Develop co-design opportunities with the community, promote engagement, and create/leverage community events for social and emotional well-being.
Networks and Peer-Led Programs: Work within the team to promote networks, peer-led programs, and social inclusion within the community.
Referral Pathways: Develop referral pathways for a broad range of health and social services, emphasizing emergency services, General Practitioners, and community health services.
Documentation and Compliance: Ensure timely documentation, maintain client confidentiality, adhere to relevant standards and legislation, and actively contribute to personal development through training.
Other requirements
Relevant qualifications or commensurate experience working in a support role for people with a mental health challenge
Excellent communication and interpersonal skills
Right to work within Australia
Current valid Driver's Licence and the ability to undertake some travel
NDIS Worker Orientation Module Certificate
Satisfactory National Police Records Check, NDIS Worker Screening Check and Working with Children's Check
