About the role
People are the heart of everything we do. As our Intake Assessment Officer, you will screen, triage, risk assess and provide high quality intake and assessment screening. The Intake and Assessment Workers will provide the intake function; receiving initial referrals, undertaking eligibility screening, and reviewing referrals to ensure the right level of care is provided.
- Receive and action all CHSP incoming referrals via my age care portal
- Review referrals with Home Care Package approvals and, provide Home Care information, and if appropriate refer on to HCP Case Manager
About You
- Knowledge and experience in undertaking assessment and case management of clients with complex health and or social needs
- An understanding of issues affecting older persons including those from special needs groups.
- Demonstrated knowledge of client pathways and services
- Excellent communication, negotiation and analytical skills.
- Excellent time management skills and the ability to work independently.
- Commitment to quality customer service
- Experience in the Microsoft software packages
- Current Drivers’ License
- Satisfactory Police Check,
- Professional knowledge and understanding of the ageing process and an awareness of the issues confronting older persons living in the community.
What's involved in the role?
This position screens incoming referrals and Support Plan Reviews from My Aged Care and conducts telephone assessments to determine an older person’s health and social needs. Duties include:
- Working professionally with clients, families and representatives, employees at all levels internally and liaising with relevant external stakeholders, including community support agencies, brokerage agencies, contractors
- Reporting to the Home Care Service Manager and liaising externally with Clients, Carers and their families, My Aged Care Contact Centre, Aged Care Assessment Service, Regional Assessment Service practitioners, Medical Practitioners and Health Professionals
- The Intake Officer will ensure a responsive and proactive intake experience for prospective clients, their families and referring agencies to provide excellent customer service to continue to grow PAC’S client base. This also includes the promotion of PAC’s services
