Warracknabeal campus is located in the heart of Warracknabeal at 18 Dimboola Road Warracknabeal, halfway between Adelaide and Melbourne.
Latitude Recruitment is recruiting an Intake Clinician on behalf of Rural Northwest Health. In this position you will work within the Intake team to provide intake support to consumers wanting to access Community Services.
This position can be part time or full time, based in Warracknabeal, and is open to those with qualifications in Allied Health or Nursing.
To be successful in this position you will have:
- Qualifications as an Allied Health Assistant, Nurse or Allied Health Professional.
- Experience in the implementation of person-directed care within a community setting.
- Good written and verbal communication skills, including computer skills, and excellent time management.
- Strong interpersonal skills and demonstrated collaboration with an interdisciplinary team.
- Strong organisation skills that reflect the ability to perform and prioritise multiple tasks seamlessly and with excellent attention to detail.
- Commitment to and willingness to participate in continued training and education related to the position.
- Knowledge of Primary Care Principles and chronic disease management principles.
- Knowledge and ability to manage Uniti.
- Demonstrated knowledge of accreditation programs and related legislation
- COVID19 vaccination evidence and immunisation history
- Current Working with Children Check or willingness to obtain one
- Current and valid Police Clearance
- Current NDIS Check or willingness to obtain one
- Current driver’s licence
