Job Overview:

As a Learn2Adult Transition Coach specializing in teenagers with disabilities, your role is pivotal in supporting young individuals in taking control of their lives and preparing for confident decision-making as adults. You will work with teenagers, families, and professionals to facilitate a smoother transition to adulthood using the Learn2Adult Decision-Making Roadmap tool.

Duties and Responsibilities:

  • Collaborate with teenagers with disabilities, their families, and other professionals to facilitate the transition to adulthood using the Learn2Adult tool.
  • Provide individualized coaching and support to teenagers with disabilities, empowering them to make their own decisions and take control of their lives.
  • Assist teenagers in developing a proactive plan for transitioning decision-making from parents to themselves, using the Learn2Adult Decision-Making Roadmap tool.
  • Guide teenagers in understanding the types of decisions they will need to make as adults and teach them the necessary skills to make informed choices.
  • Support teenagers in building self-confidence, self-awareness, and self-advocacy skills to navigate adulthood successfully.
  • Collaborate with families to ensure a smoother and less stressful transition, fostering an adult-adult relationship between parents and their adult children with disabilities.
  • Stay informed about the unique challenges and needs of teenagers with disabilities transitioning to adulthood and integrate this knowledge into coaching strategies.
  • Monitor and assess the progress of teenagers using the Learn2Adult tool, adjusting coaching strategies as needed to ensure positive outcomes.
  • Promote a better quality of life for teenagers and young adults with disabilities, increasing the protection of their human rights.
  • Work towards reducing behaviors of concern and minimizing the use of restrictive practices through effective coaching and support.

To be successful in this position you will have:

  • Possession of a Certificate II in Disability or a related field such as Special Education, Psychology, or Social Work is preferred.
  • Experience working with teenagers with disabilities, preferably in a coaching or support role.
  • Familiarity with the Learn2Adult tool and its application in facilitating the transition to adulthood for individuals with disabilities.
  • Knowledge of disability rights, self-advocacy, and person-centered planning approaches.
  • Excellent communication and interpersonal skills to effectively engage with teenagers, their families, and other professionals.
  • Empathy, patience, and the ability to establish rapport with teenagers with disabilities and their families.
  • Strong organisational and time management skills to facilitate productive coaching sessions and maintain accurate documentation.
