LIFESTYLE SERVICES OFFICER
***You will be required to assist out in the field when situations arise to support our participants and be available to occasionally take on our after hours on-call enquiries***

Working as part of a disability support team the Lifestyle Services Officer will support the delivery of support services to ensure the achievement of Participant’s goals and objectives.
For this position, you'll need to thrive in a busy, fast paced environment, have fantastic interpersonal skills and love collaborating with other divisions to create exceptional client experiences.
This is a great opportunity for a person who has Individual Support worker experience looking to diversify their skills and step up. You will provide office support and manage activities related to Service Delivery (setting up meetings, scheduling appointments, sorting documents, etc.). Additionally you need to be prepared to undertake general reception, administration, and housekeeping duties.
Some of your key responsibilities will be to:
- Support and monitor the update of all care plans and risk assessments
- Ensure that all documentation is implemented and captured in line with company process for SIL houses
- Ensuring timely closure of incident reports identified within position scope
- Support all administration aspects of service delivery team, inclusive but not limited to ordering SIL consumables, filing of medication and house documents, supporting new SIL house roll-outs
- Support Participant targets and outcomes within all operational areas / programs
- Organise and administrate where required, team training and onboarding training in accordance with client care needs and plan goals
- Work on call as well as with On-Call personnel on specific rostering and service delivery matters
- Ensure services are provided so as to adhere to relevant industry legislation and standards
Essential
- Experience following a person-centred support model to facilitate the development of independence, confidence, self-esteem, and social interaction skills in people with disability
- Commitment to social justice and inclusion and valuing difference and diversity.
- 2 years’ experience working in Disability or Community Services sector
- Demonstrated understanding of disability, mental illness, and the needs of carers
- Experience engaging with people who may have complex needs and behaviours
- Strong coaching and mentoring skills
- Knowledge of and experience working within the Social, Community, Home Care and Disability Services (SCHADS) Industry Award 2010
- Being adaptable to changing circumstances and being able to prioritise work.
- Strong organisational and administrative skills including data management, record keeping and reporting
- Ability to work as part of a team with limited direct supervision
- Well-developed communication, interpersonal and advocacy skills including the ability to work and communicate effectively with managers, office staff and Lifestyle facilitators.
- Ability to develop and maintain linkages with a diverse range of service providers and client groups
- Understanding of privacy and confidentiality issues.
- Ability to compile operations report for monthly reporting.
- Ability to manage time and awareness of fatigue
- Queensland Provisional Green or above Driver’s License
- Ability to obtain positive notice in the Working with Children Check (Blue Card) and Disability Worker Screening (Yellow Card)/NDIS worker screening
- NDIS Worker Orientation Module
- Nationally recognised First AId & CPR Certificate
Desirable
- Experience working within Supported Independent Living (SIL) residential environments
- Experience working with DataNova FlowLogic system would be highly advantageous
