The Opportunity
We are looking for a Linkages Care Coordinator to join our supportive and professional multidisciplinary teams on a 12 Month contract 0.4FTE.
Responsibilities include but are not limited to;
- Triaging of referrals and processing of initial assessments to the Linkages Program
- Develop individual client care plans
- Maintain comprehensive up to date client records,
- Facilitate referrals to My Aged Care for age-appropriate Linkages clients
- Supporting NDIS applications for eligible Linkages clients
- As required, attend to any other administrative duties associated with the program and meeting any funding targets and deadlines
- Work in accordance with the policies and practices of the Linkages Program.
- Promote awareness and understanding of the Linkages Program with all internal and external stakeholders
Skills & Experience
- A relevant tertiary qualification -Social Work, Allied Health, Nursing, or Community Services and or relevant experience Demonstrated commitment to the philosophy and principles of Community health and a positive view of working with complex and marginalised people,
- Case Management/ Care Coordination of vulnerable and/or marginalised people in a community setting
- Extensive experience and high level of skill in working with children and/or adults with disabilities, and their carers, including those from a CALD background,
- Demonstrated ability to use broad community networks, knowledge of Linkages Program, and the NDIA eligibility criteria and access processes,
- Knowledge of community-based service provision and systems
- Demonstrated skill in setting and monitoring client budgets,
- Ability to manage service agreements for regularly brokered services
- Current driver’s licence
