This role pertains to aged care. We will perform reviews 3- to 6-monthly and write individualised care plans for our patients.
Role Overview: We are building a broad team of Allied Health Professionals to join our consulting team. You will work as part of a multidisciplinary team, conducting clinical reviews and consulting on priority patients in aged care principally utilising medicare funding.

This role typically involves in-person or remote work, with flexible hours, allowing you to make a meaningful impact on patient care from the comfort of your own location.
Typical workdays will comprise 3.5-4h AM and PM sessions, including meetings and some patient consultations. Team members may consult for a single session only, allowing for flexibility.
Key Responsibilities:
- Participate in multidisciplinary team meetings to review and manage care plans for aged care patients.
- Provide expert advice and consultation on patient management and clinical reviews.
- Collaborate with other Health Professionals to ensure a comprehensive and integrated approach to patient care.
- Utilise virtual platforms for patient consultations and team meetings if working from home or remotely.
- Maintain accurate and up-to-date patient records and documentation.
Requirements:
- Relevant qualifications and experience as an Allied Health Professional
- Experience in aged care or a related field is desirable.
- Strong communication and teamwork skills, with the ability to collaborate effectively in a virtual environment.
- Proficiency with digital tools and platforms for virtual healthcare is desirable.
- Ability to work independently and manage time effectively in a remote setting.
- Full AHPRA registration and a dedicated Medicare provider number are essential.
