What you'll be doing
The Transitional Aged Care Program (TACP) Service Team Leader will provide service co-ordination, clinical leadership and administrative co-ordination of the Transitional Aged Care Program.
They will ensure consistent, efficient and effective day to day running of the TACP program to residents of WSLHD.
They will ensure TACP therapists and teams work efficiently and provide high quality care.
