Job Description:
We are seeking dedicated and experienced Mental Health and SIL Support Workers to join our team at swara. In this role, you will provide personalised support to participants with complex Mental Health conditions, helping them achieve their goals, enhance their quality of life, and maintain their independence. You will work closely with participants, their families, and a multidisciplinary team to deliver holistic and person-centered care.
Key Responsibilities:
- Provide one-on-one support to participants in their daily activities, promoting independence and well-being.
- Develop and implement individualised care plans in collaboration with participants and the multidisciplinary team.
- Assist participants with personal care, medication management, and healthcare appointments.
- Facilitate social, recreational, and educational activities to enhance participants social skills and community engagement.
- Monitor and document participants progress, behaviour, and any changes in their Mental Health status.
- Support clients in developing coping strategies and life skills to manage their Mental Health conditions.
- Ensure a safe, respectful, and non-judgmental environment for participants at all times.
- Communicate effectively with participant, their families, and the stakeholder team
Qualifications:
- Certificate IV in Mental Health, Community Services, or a related field
- Minimum of 2 years’ experience working with individuals with complex Mental Health needs
- Strong understanding of Mental Health disorders, disabilities and their impact on daily living
- Valid driver’s license and reliable transport
- Current First Aid and CPR certification
- BlueCard/NDIS Screening Card (or willingness to obtain).
