Bayside Acute Care Team (ACT) Metro South Addictions and Mental Health Service are looking for a Multi-disciplinary Team Leader to join their Bayside Community Health Centre team. The purpose of this role is to provide clinical expertise and consultancy through advanced clinical leadership relevant to the speciality of Mental Health.

About the Role
HP5:
- Facilitate evidence-based practice in the area of interprofessional team members within Bayside ACT to ensure efficiency and effectiveness of service provision and discharge planning, through implementation and facilitation of quality initiatives, literature reviews, attendance at professional development activities and involvement in research and education.
- Lead and support the delivery of clinical services in accordance with accepted professional practices, standards and principles including the legal and ethical requirements with a high degree of independence.
- Apply advanced level independent professional judgment regarding the co-ordination and treatment planning of complex patients, as it relates to case management, discharge planning and service transition.
Nurse Grade 7:
- Responsible for clinical leadership for Bayside ACT services through the application of advanced clinical knowledge, skills and competency according to contemporary practice and standards of care.
- Maintain high level of interpersonal and negotiation skills to inform and train the healthCare Team of clinical practice updates and research findings within Bayside Community Mental Health area.
- Utilise advanced clinical skills and expertise to support nurses in patient care planning, delivery and education.
- Collaborate and supports CNC and the healthCare Team to deliver safe clinical practice in alignment with organisation goals and unit priorities.
- Responsible for the content expertise for Bayside Community Mental Health specialty to provide the highest standards of safe quality care and clinical nursing practice to meet the requirements for person centred care.
- Demonstrate critical thinking and reasoning, advanced problem-solving skills and expert clinical judgement in the provision of contemporary nursing practice.
- Provide expert person-centred practice in the provision of direct and indirect patient care across the care continuum.
