About the role
Mercy Community has an opportunity for an experienced and passionate a Specialist Support Coordinator. This is a Permanent Full-Time position based in Toowoomba
Specialist Support Coordinator provides supports, coordination and assistance for NDIS participants of higher-level complexity including managing challenges in their support environment and overcome complex barriers that affect participants ability to access and maintain supports.
$93,860 - $98,108 annually (SCHADS L5) plus superannuation plus generous salary packaging available
What you’ll be doing
In this role your day-to-day responsibilities will include but are not limited to:
- Supporting and communicating directly with participants, their families and other support networks to provide information and understanding of their NDIS plan
- Connect, establish, and maintain participants with support services to pursue/achieve their goals
- Plan and coordinate participants to access support services in accordance with their allocation NDIS funds/budgets
- Advocating for and supporting participants to communicate their goals and aspirations to their stakeholders and the National Disability Insurance Agency (NDIA)
- Complete and provide reports to the NDIA with outcomes and success indicators relating to the participants plan/goals
- Deliver Specialist Support Coordination/Complex Support Coordination in line with the NDIS Quality and Safeguards commission guidelines/requirements
About you
To be successful in the role, you will possess:
- Experience as a NDIS Specialist Support Coordinator (highly desired)
- Hold a qualification in a relevant field to Disability, Community or Healthcare Services (highly desired)
- Hold a current Queensland Blue Card (or willing to apply)
- Hold a current NDIS Workers Screening Clearance (or willing to apply)
- Hold a current QLD Drivers Licence and have your own vehicle (mandatory)
