About You
Experienced in supporting people with mental disorders you will be motivated by seeing these individuals thrive within their communities as a result of care plans that promote independence, resilience and confidence.
This will have been gained in a role with similar case management responsibilities where you have supported a portfolio of highly complex clients in helping them to access supports and services within their community.
To achieve this, you will possess:
- Completed Degree qualifications in an Allied Health discipline with experience in Mental Health or other related disciplines (mandatory min. level of qualification)
- Experienced working in Support Coordination, as a Local Area Coordinator (LAC) or Care Plan Coordination
- An understanding of the Trauma Informed Care Network as well as NDIS Framework
- Knowledge of accessing community resources, particularly in the areas of health, drugs and alcohol, legal, income support, employment and recreation
- The ability to approach problem solving with creativity, resourcefulness and wisdom
- Effective written, computer literacy and verbal communication skills

About the Role
As a Specialist Support Coordinator for ermha you will actively manage a portfolio of clients and provide coordination in accordance to their NDIS plan, assisting to connect with supports, build capacity and resilience. You will also provide support, supervision and guidance to staff.
To achieve this the role entails:
- Managing complex clients for support coordination program
- Developing support plans & Implementing NDIS plans
- Report writing (including NDIS & MACNI reports)
- Making appropriate referrals to external services
- Monitor funding
- Chair/minute care team meetings and provide supervision to staff
- Conducting needs assessments based on their NDIS plan and goals
- Care plan coordination for NDIS participants that also have MACNI support
- Liaising with stakeholders (NDIA, DHHS, MACNI, clinical care teams, case managers, allied health professionals & local service providers etc.)
- Assisting clients to achieve goals through building capacity, knowledge, resourcefulness and confidence
