Role Description
YAY Today are currently expanding our Support Coordination division and are seeking an experienced and highly motivated Support Coordinator to join our team.
YAY Today’s Support Coordinators provide coordination of support to NDIS participants to assist them in achieving goals set out in their NDIS plans. This role has a key focus on supporting participants to direct their lives, not just their services, and is focussed on assisting participants to build and maintain a resilient network of formal and informal supports. As a Support Coordinator, you will be responsible for strengthening a participant’s ability to design and build their supports with an emphasis on linking the broader systems of support across a complex service delivery environment.
This role is Hybrid, with WFH and 2 days in our office located in North Rd, Ormond, and occasional participant in-home visits. The Employer is welcome to flexible working arrangements should this be required. Duties & responsibilities:
- Working with participants to achieve their goals in line with their NDIS plan.
- Providing ongoing Support Coordinator services to participants in line with their NDIS plan.
- Utilise a person-centred approach to build capacity, choice, and control for the participants.
- Develop and implement support plans with clients and monitor their progress and outcomes.
- Identify and provide a wide range of solutions to complex challenges, barriers and risks that participants face.
- Undertaking work in line with the organisational policies, procedures, and values.
- Complying with all relevant legislation and codes of conduct.
- Maintaining an up-to-date knowledge of local services and organisations participants can connect with
Experience & qualifications:
- Minimum of 1 year experience in NDIS support coordination
- Excellent communication skills (verbal & written).
- Qualifications in community services, disability, or other health related disciplines, minimum of Cert 4.
- In-depth knowledge of the NDIS rules and operational guidelines.
- NDIS worker screening clearance & blue card.
- First Aid and CPR certifications.
- Excellent interpersonal skills to foster productive working relationships with an array of stakeholders.
- Permanent Australian working rights & current residence.
- Computer literacy and proficiency in Microsoft suite.
- Ability to work autonomously and take initiative.
