Keep Connected Support Coordination is expanding and looking for a passionate and hard-working person to join our team! This is a strictly work from home role however occasional travel to meet participants and stakeholders will be required. Keep Connected is a small independent provider located in South East Melbourne. We are a registered NDIS provider catering to all NDIS participants for Support Coordination and Specialist Support Coordination (Level 2 and 3). Qualifications & experience
- Tertiary education in either Disability, Community Services, Social Work, Psychology or substantial industry experience
- NDIS Worker Screening Clearance
- Current Drivers License
- Working with Children’s Check
- Previous experience working as a Support Coordinator or in a case management role
- Strong knowledge of the disability and community services sector
- Experience working with people with disabilities, child protection involvement, homelessness, Alcohol and Other Drugs and mental health
- NDIS Online Orientation Module
Tasks & responsibilities
- Assist clients to exercise choice and control by understanding and implementing their NDIS plans
- Support clients to connect to NDIS and mainstream services
- Support participants through planning meetings and reviews
- Adhere to thorough case note recording and prepare NDIS reports as required in line with support coordination requirements
- Meet weekly KPI targets set out by Keep Connected
- Work autonomously with strong organizational skills and self motivation
