NDIS Support Coordinator (PT 0.6 FTE) - Adelaide
About the organisation and the role
Huntington’s Australia is a national leader in the delivery of services to support and empower people impacted by Huntington’s Disease (HD) to live their best life. We provide NDIS services, community education, individual and group capability building activities and advocacy at all levels.
This role is part of the national NDIS Services team within the Community Programs & Services function. Based in our Adelaide office, and reporting to NDIS Services Team Leader the provides support coordination to people with Huntington’s Disease (HD) in South Australia and the Northern Territory to activate and manage their NDIS plans and engage with a range of mainstream and disability providers to optimise outcomes and achieve their NDIS plan goals.
The role is covered by the Social, Community, Home Care and Disability Services Industry Award and is classified at Grade 5.
About You
This challenging role will suit someone with experience in NDIS Support Coordination coupled with a background in social work, community support and/or assisting people with neuro- degenerative disorders.
- Building on your extensive experience and knowledge of the NDIS, you will have a good understanding of associated process and support networks and the ability to deal with changes in the disability and aged care sectors.
- You will have well-developed interpersonal skills, utilising an empathetic approach to establish and maintain supports for individuals impacted by HD.
- Your ability to build trust with clients who may have complex physical, social, and psychological care needs is essential.
- Your high level written, and verbal communication skills will be backed up by the ability to negotiate with, and influence service providers and stakeholders on behalf of participants.
- Good planning and organisational skills will be essential as you work effectively to achieve deadlines and balance priorities.
- Your ability to develop effective working relationships with health and community service providers is essential.
