1. Participant Engagement:
- Engage with individuals with disabilities to understand their needs and goals.
- Conduct assessments to determine eligibility and support requirements under the NDIS.
- Collaborate with participants and their support networks to develop personalized NDIS plans.
- Ensure plans align with NDIS guidelines, funding constraints, and participant preferences.
3. Coordination of Services:
- Liaise with service providers to arrange and coordinate support services according to participants' plans.
- Monitor service delivery to ensure it meets NDIS standards and participant expectations.
4. Documentation and Record Keeping:
- Maintain accurate and up-to-date records of participant information, plans, and progress.
- Prepare reports and documentation required for NDIS compliance and reporting
- Advocate for participants' rights and needs within the NDIS framework.
- Address any issues or concerns that may arise during the implementation of NDIS plans.
6. Stakeholder Collaboration:
- Work closely with relevant stakeholders, including families, carers, and healthcare professionals.
- Foster positive relationships with service providers to enhance the overall support network.
7. Compliance and Quality Assurance:
- Ensure compliance with NDIS policies, procedures, and regulations.
- Implement quality assurance measures to enhance the effectiveness of support services.
8. Training and Education:
- Provide information and education to participants and their families about the NDIS process and available services.
- Stay informed about changes in NDIS policies and communicate updates to stakeholders.
- Address any issues or conflicts that may arise during the implementation of NDIS plans.
- Seek solutions to overcome barriers and optimize support for participants.
10. Community Engagement:
- Participate in community events and initiatives to promote awareness of the NDIS.
- Contribute to the development of a supportive and inclusive community for individuals with disabilities.
