About the Role
At Abito Care, NDIS Support Coordinator assesses needs and coordinates support for NDIS participants.
Your key responsibilities as a NDIS Support Coordinator includes:
- Assist NDIS participants to navigate the NDIS System, advocating for their individual needs, promoting person-centred outcomes for Participants to support their individual aspirations and identified goals, and coordinate their services and supports.
- Develop individualised support plans and support Participants in implementing their NDIS Plan to maximise the value for money through their supports.
- Complete all necessary administrative tasks relating to this role (including monitoring and assessing progress towards Participant's goals, regularly updating reports and sharing updates with Participants and their families) to the required standard, ensuring full compliance.
- Maintain and develop relations with Participants, their families, service providers, NDIS Planners, and other non-NDIS government bodies.
- Support Participants to connect and engage with providers, to access supports, and build Participant's capacity to maintain these relationships.
- Take ownership for developing and retaining these networks.
- Promote Abito Care to the market and generate interests in support coordination under the NDIS. This will involve liaising with Local Area Coordinators, Disability Service Providers and other support groups including NDIS Participants and their families/nominated representatives.

You’ll also need to:
- Have Minimum 6 months to 2 years experience working as NDIS Support Coordinator
- Have Allied Health qualifications; or minimum Certificate IV in Allied Health/Disability/ Age care or diploma or university qualification or similar
- Have experience or knowledge in complex families health issues would be beneficial
Successful applicants will be required to have or undergo a police check, NDIS Worker Screening Check and will have to provide a current Working with Children Check.
