Project Make Lemonade are currently recruiting a passionate, organised and experienced Support Coordinator to join our team at Currajong (Townsville). Project Make Lemonade is part of the Laydigo Group. A short video about our owners can be viewed below.
What we need from you
- Provide support coordination, and/or psycho-social recovery coaching, to NDIS participants with diverse support needs.
- Utilise a person-centred approach to build capacity, choice and control for the participants
- Support participants to understand and use their NDIS Plan
- Connect participant with NDIS Providers, community, mainstream and government services
- Build the participants capacity to manage their NDIS Plan in future plan periods
- Coordinate services – making calls, attending meetings, and providing support to participants, families, care staff and support networks as required
- Support participants current service providers
- Ensure record keeping is up to date, efficient and sufficient
- Ensure the use of NDIS funding meets Reasonable and Necessary criteria as per NDIS Act 2013
Experience & qualifications
- Minimum of 2 years’ experience in NDIS support coordination
- Qualifications in community services, disability, or other health related disciplines, minimum of Cert 4
- In-depth knowledge of the NDIS rules and operational guidelines
- Current driver’s license & your own fully insured and safe vehicle
- NDIS worker screen clearance
- First Aid and CPR certifications
- Excellent interpersonal skills to foster productive working relationships with an array of stakeholders
- Permanent Australian working rights & current residence
- Computer literacy and proficiency in Microsoft suite
- Ability to work autonomously and take initiative
