AddressConnect4U Australia is a NDIS-registered, independent provider of Level 2 and Level 3 support coordination. We are seeking an outcome-focussed Support Coordinator (contractor position) to join our team. The successful applicant must live in the Brisbane North to Caboolture corridor and have good local knowledge and connections. Support Coordinator experience preferred, or solid transferrable skills from professional positions. Lived experience is also preferrable.
The primary objectives of the role include assisting NDIS participants to:
- Implement their plan and increase their capacity to manage/direct their own supports.
- Have greater opportunities to explore and connect with their community and alternative support options.
- Strengthen their informal support networks.
- Maximise the value for money they receive from their supports.
- Access and use the NDIS Participant Portal.
- Effectively communicate and maintain relationships with the NDIA.
The position offers work/life balance with work from home and flexible hours to be completed on a full-time equivalent basis. Support Coordinators are mentored by dedicated Managers who are experienced practitioners in the support coordination field.
