About the role
Mercy Community Disability Service team have an opportunity for a passionate NDIS Support Coordinator. This is a rare Permanent Part-Time (3 days per week) position located in Gympie (you will work from home with some travel to participants as required).
A Support Coordinator works directly with people living with disability and their families, to coordinate, manage and maximise their NDIS plan. You will truly make a difference in helping to enable greater independence across all domains of an individual's life.
$82,044 - $88,288 annually (SCHADS L4) plus superannuation plus generous salary packaging available
What you’ll be doing
As a Support Coordinator, your key duties will include:
- Support participants to understand and create their NDIS plan, goals and funding access
- Plan and coordinate participants to access support services in accordance with their allocated NDIS funds/budgets
- Connect, establish and maintain participants with support services to pursue/achieve their goals
- Use NDIS systems and processes to achieve the most of NDIS plans for the participant
- Montior, support, track and review participants NDIS plans/goals
- Complete and provide reports to the NDIA with outcomes and success indicators relating to the participants plan/goals
About you
To be successful in the role, you will possess:
- Experience as a NDIS Support Coordinator (highly desired)
- Hold a current Queensland Blue Card (or willing to apply)
- Hold a current NDIS Workers Screening Clearance (or willing to apply)
- Hold a current QLD Drivers Licence and have your own vehicle (mandatory)
