Lifestyle Plus is a NDIS registered provider located in Adelaide providing both CORE disability support services and Support Coordination along with support for those in Aged Care.
Established in late 2015, Lifestyle Plus is a private company dedicated to providing support services to a diverse community. Our reach extends to individuals with disabilities, those over 65, and anyone in need of support. Currently, we support participants with NDIS plans, and we also serve private aged care clients who value our services.

We are currently looking for a Level 2Support Coordinator to join our growing team.
Our ideal candidate possesses:
1 - In-Depth NDIS Knowledge
- An exceptional understanding of the NDIS, including core principles, legislation, and rules
- Demonstrated comprehension of the NDIS Price Guide and the ability to work flexibly within budget constraints
2 - Effective Time Management and Documentation Skills:
- Exceptional time management skills for efficient coordination of NDIS support plans
- Proficiency in using CRM systems, specifically CareView and Microsoft Office, for accurate and timely record-keeping of activities
- Strong computer literacy and an ability to quickly adapt to new software programs or tools as required
- Proficiency in maintaining clear and accurate case notes and other records within our systems.
3 - Strong Interpersonal Skills:
- Excellent interpersonal skills enabling the building and maintaining relationships with various stakeholders. This includes internal and external parties such as other service providers, participants' families, and plan managers. These skills are integral to fostering cooperation and collaboration among all involved parties
- Strong verbal and written communication skills to collaborate effectively with Participants, their families, Guardians and various stakeholders. This proficiency ensures that participants recieve the support they require while retaining their autonomy and control over their choices in the support they receive.
4 - Proven Experience:
- A track record of success in a similar role within the NDIS or related fields.
- Prior experience in a a Support Coordination role or similar NDIS/LAC/Service Coordination/Case Management position
- The ability to support the engagement of plan managers to establish appropriate claim categories and attribute the correct amount of funds where applicable
- The ability to work autonomously while also collaborating with a small, experienced, and dedicated team in our office, contributing to a cohesive and supportive work environment.
An ideal candidate meeting these criteria is expected to make a significant and positive impact on our organisation and the individuals we support within the NDIS framework.
Desirable Requirements:
- Certificate III in Disability or Individual Support
- DHS Disability Clearance/NDIS Worker Screening check
- Current Driver's Licence
- Unrestricted Working rights in Australia
- NDIS Worker Orientation Module
- Fully Vaccinated (2 doses + booster of a TGA Approved Vaccination)
- Do you have a current First Aid & CPR certificate
