The RoleAs a CoAbility Support Coordinator, you will be pivotal in providing exceptional support services to Level 2 and 3 participants. Your work will directly impact and make a real difference in our participants' lives. Your main responsibilities will include:
- Connecting participants with relevant services and supports
- Helping participants understand and make the most of their NDIS plan
- Helping participants navigate challenges in their support environment, such as access to education, health, justice services and similar
- Reducing barriers that prevent participants from implementing or using their NDIS plan as intended
- Helping participants understand and negotiate service agreements
- Helping build participant capacity and self-advocacy skills
- Developing and designing support solutions to meet participant outcomes
- Assisting in resolving points of crisis and developing capacity and resilience within the participant's network
- Coaching participants through challenges
- Collaborating with a variety of stakeholders
- Assisting participants in preparing for their plan review and reporting their achieved outcomes
- Modelling and reinforcing organisational values, culture and practice
To excel in this position, you will:
- Have a Tertiary qualification in social work, human services, community services or other relevant discipline
- Have lived or learned experience and the ability to demonstrate transferable skills
- Manage competing priorities and deliver quality outcomes when working under pressure in a remote environment
- Embrace cultural awareness and inclusivity as integral components of professional practice
- Have well-developed interpersonal and communication skills, both written and verbal, including the ability to connect with participants in a professional and empathetic manner
- Have comprehensive operational knowledge of the NDIS and relevant legislation, policies, regulations, and service benchmarks.
- Be process-oriented with excellent organisational and time management skills
- Demonstrate a strong work ethic and commitment to excellence, including a proven ability to meet KPIs
- Demonstrate an ability to network and develop sound relationships with local service providers
