About the opportunity
We are currently looking for a Support Coordinator who is dedicated and passionate about making a difference.
As a Support Coordinator you will support our customers to connect with service providers and achieve the goals in their NDIS plan. This involves:
- Empowering NDIS participants to understand, implement and review their NDIS plans.
- Facilitation of NDIS participants, nominees and/or persons responsible to build capacity to manage their own NDIS plans and exercise choice and control over the supports received to assist the participant to achieve long term and short terms goals.
- Building and maintaining relationships with a range of internal and external stakeholders in order to secure suitable support for individuals living with a disability.
- Assisting with plan reviews, developing reports and arranging quotes with service providers.
- Working with participants and providers, negotiating, reviewing and monitoring services.
- Developing and maintaining relationships with NDIS participants, nominees, families, providers, clinicians and government agencies.
- Work within set KPI’s

About you
We are looking for a friendly, compassionate and dedicated person who wants to have a positive impact on the lives of our customers and their families.
This full-time, 12 month fixed term maternity leave cover role will be based in Ballarat. You will require strong organisational and time management skills as well as:
- Tertiary education in Psychology, Social Work, Allied Health or similar.
- High level verbal and written communication skills.
- Experience working within a holistic practice framework with other providers/supports to achieve the best outcomes for clients.
- Experience managing complexities, while modelling an ethical customer-focused practice.
- Experience mentoring less experienced support coordination staff
Essentials
- A degree in Social Work, Psychology or other relevant qualification and experience/lived experience supporting individuals with complex needs is highly regarded.
- Relevant experience empowering and supporting people with an intellectual disability and/ or children and young people in a related area/ field
- Relevant experience in coordinating responsive, individualized support to participants with complex needs and their families within the NDIS.
- A commitment to person-centred and family centred practice and maximizing the opportunities and support of participants with disabilities and their families within their local communities.
- A sound understanding of relevant legislation, NDIS procedural guidelines and the full range of community services that may be required by participants.
- Awareness of the rights of people with intellectual disabilities and/or multiple disabilities;
- Strong commitment to inclusion of people with disabilities within the community;
- Demonstrated interpersonal skills with the ability to communicate and relate to people we support and their families and friends.
- Demonstrated ability to work as part of a team and contribute to team goals.
- Advanced written and verbal communication skills.
- Competent administrative and documentation skills
- Maintain current driver’s licence;
- Hold a current NDIS Workers Screening Check and Working With Children's check.
