We are seeking either an experienced NDIS Specialist Support Coordinator or an experienced SupportCoordinator to join Brightwater’s Capacity Building Program. You will be part of a professional and skilled team, where team culture and great client outcomes are at the core of everything we do.
This position is offered on a permanentpart time basis and provides you with the work life balance of working 3 days per week (45 hours per fortnight). You will enjoy choosing to work flexibly between our Inglewood, East Victoria Park and Marangaroo offices, and also from home.
Our Support Coordinators have a vital role in bringing together clients and their families with the people and services that are important to them on their NDIS journey. Your knowledge and experience in supporting Participants to develop their capacity for independence and to optimise their goals and plan funding and some ‘out of the box’ thinking, will be highly valued.
As a Specialist Support Coordinator, you will support clients to navigate through more complex situations; to reduce risk and facilitate sustainable partnerships across a mental health, acute, housing transitions, homelessness and social supports; both mainstream and funded services.
In joining our team of 9 Support Coordinators (SSC and SC), you will also have the support of being part of a broader therapy and behaviour support team. We prioritise regular team meetings and supervision to upskill and share ideas because we choose to work with adult clients who have complex needs; mainly neurological and neuro-degenerative conditions; and with special interests in Younger Onset Dementia and Huntington’s disease.
This position is required to work in the community, and as much as possible locally to your client base. There is a significant amount of travel for which you will be reimbursed so a reliable car, and willingness to optimise the way you schedule you visits and, where possible, to use our fleet vehicles, is an expectation of the role.
