About The Role
As a Specialist Support Coordinator with Propel, you will assist our clients and their families to access services through the NDIS. You will use a person-centred approach to:
- Identify and foster a support network for our clients that meets their needs, short-term and long-term goals.
- Advocate for and build confidence in our clients, assisting them to understand and self-direct their NDIS plan.
- Liaise with and manage stakeholders to establish supports and services for our clients tailored to their individual needs.
- Assist clients with revisions and alterations to their NDIS plan and service providers.
- Manage and optimise funding and resourcing of client plans to ensure they receive the best available services.
- General administration including reporting, billing, record keeping and taking client enquiries.
We are looking for candidates seeking part time or full time employment.
Requirements
- 2+ years demonstrated experience in support coordination or other relevant experience with transferable skills
- Demonstrated experience working as a Specialist Support Coordinator managing complex situations such as housing, behavioural issues, mental health concerns, forensic involvement, Child Protection and Family Services and/or other significant issues
- Experience working with people with a disability or mental health issues to support them to foster positive relationships with service providers
- Excellent verbal communication to understand and discuss NDIS participation with clients and their families
- Strong written and computer skills for the creation of plans, programs, reports, and other documents
- Strong time management and prioritisation with the ability to problem solve on your feet
- Ability to work independently using initiative, as well as part of a team to support the best interests of clients
- Valid and current National Police Check and Working with Children Check (WWCC)
- Valid and current NDIS Worker Screening Check and completed NDIS Worker Orientation Module
- Unrestricted Driver’s Licence
Desirable
- Tertiary qualification in Social Work, Disability, Individual Support, and/or Community Services
- Current First Aid/CPR Certificate
